Who should choose what position a kid plays, coach, child or parent? Does this change at different age / ability level?

What age? Under 12 B kids should moved around and play and learn more multiple positions with the exception of goalie. 12A and higher they should be pretty set at what they are. Find their strengths and go from there, being a two-way player is the ideal thing to be.

Don't matter age and level 3 things remain the same. Coaches coach, players play and parents bitch and moan. Let the coaches do their thing, even more so at AAA. You should have a paid coach that knows what they are doing not a parent coach just filling the spot.

Peewee and below, for the most part unless they are a goalie, should be playing every spot. Or get some experience at every spot.
You know your kid who is the leading goal scorer at U10 should most def play and learn defense. Because goals dry up real quick as they get older.

You ever get bored take the USA Hockey coaching certification and you really see how nobody follows the development path in youth hockey.  It is very eye opening and you learn a lot of information.  It also covers your specific question.  They give you everything how to develop a player.  Coaching development should be more of a priority for clubs you would get a better product!
